1st Appointment for New Patients
Welcome to Stonecreek Family Physicians. On this page we have several pieces of information that will benefit you as new patient to our practice.
The first of which is our New Patient Information handout. Please take the time to read this information as it will help you understand how our office works and explains the forms we need you to complete and bring with you to your first appointment.
Also important to read, is our Notice of Privacy Practices which explains how we will use and disclose your protected health information and your rights associated with the sharing of this information.
Once you have read the New Patient Information and Privacy Practices, you will be ready to gather complete the forms for your appointment. We have two sets listed below based on age.
Go to Download Patient Forms to print out and bring in your appropriate age registration form.
When you show for your appointment, please arrive 30 minutes early and bring the following with you:
- Medical History Form
- Financial Policy
- Insurance card(s)
- Immunization records - This is important as we will not be able to administer any shots without this information.
- Medication List
- Patient Registration Form (This form will be given to you when you arrive for your appointment)
